Drowning Incident Report

Per the Centers for Disease Control and Prevention (CDC), drowning is the leading cause of death for children between the ages of 1 and 4 in the United States.

To assist in protecting the health and safety of all Pennsylvanians, the Bureau of EMS (Bureau) is collaborating with the Bureau of Community Health Systems (BCHS) to investigate drowning and near drowning events within the Commonwealth as required by BCHS regulations. To accomplish their requirement of having an Environmental Health Specialist complete an operational inspection at regulated facilities, BCHS needs to be made aware whenever a drowning or near drowning event occurs in Pennsylvania.

The Bureau is asking EMS agencies to complete the attached form within 72 hours of a drowning or near drowning event.  Completion and submission of the electronic form below will automatically submit your information.
